Long Term Care / Case management

Case Management (also known as Care Management) is a relatively new concept. It has the overall aim of empowering and assisting the disabled client and their family to organise or co-ordinate aspects of their care. The role of Case Manager will vary depending on the needs of the client but can include may of the following aspects:

Care at Home

  • To develop care plans to meet client’s holistic needs.
  • Registered with the Care Commission to recruit neccessary nursing, care and domestic staff to maintain the care plan.
  • To undertake payroll functions on behalf of the client.
  • To assist and support clients moving from hospital to the community and provision of immediate needs assessment.
  • To assist and support clients transition from family based care regime to employed care regime.
  • To meet staff and family training needs and review this regularly.
  • To implement systems to monitor the quality of care/support provided.

Occupation

  • To help facilitate integration or reintegration to employment.
  • To assist client to plan and organise meaningful activity to provide ‘occupation’ if return to employment is not possible.

Leisure and Recreation

  • To assist clients to plan and organise a structured programme of leisure and recreational activities.
  • To assist clients to seek necessary resources to persue new recreational activities.

Resources

  • To work with client to make applications for statutory benefits.
  • To assist client with the acquisition of specialist equipment such as wheelchairs.
  • To assist with the acquisition and co-ordination of necessary health professionals such as occupational therapy, physiotherapy, etc.
  • To work with client to identify any needs for alternative housing or housing adaptions.

Communication

  • To co-ordinate services, statutory, voluntary and private and facilitate communication between the services.
  • To liaise and communicate with legal personnel involved with the client.

Clients can be funded by Personal Injury Litigation Claims. The claimants solicitor can often obtain an interim payment for services such as case management, rehabilitation, care and equipment. Services are also available for Health Trusts or Social Work Departments to purchase for their service users through direct payments.
